Needle nose pliers or something similar and someone with a strong grip will be needed to untwist the top die if it has jammed in the table press. It might help if you could wrap something around the die so the pliers can get a better grip without slipping -- something like those silicone pot holders that can also be used as jar openers. Even fabric could be helpful.
Going forward, it's recommended that you always leave the top die ( any top die) a little bit unscrewed so that 2-3 threads are still visible. This will help to prevent the die from jamming with excessive pressure.
